The City of Grande Prairie, in partnership with the Grande Prairie Primary Care Network (PCN), invites residents aged 55 and older to attend upcoming information sessions to learn about the benefits of Social Prescribing — a community-based initiative designed to enhance overall well-being by connecting individuals to non-medical supports.

Social Prescribing was launched earlier this year as a collaborative initiative between the City and PCN to support older adults facing social isolation, mental health challenges, or difficulties accessing essential resources.

What Social Prescribing Offers:

Through personalized guidance, individuals can access a wide range of community supports, including:

  • Social groups and recreational activities
  • Transportation services
  • Meal programs and in-home support
  • Health and wellness opportunities
  • Financial and housing resources
  • Lawn maintenance and snow removal assistance through the Council on Aging
